Waiver and Release of Liability and Indemnity Agreement (Minor Participant)
I know that running in an event that is organized as a virtual activity where my child runs on his/her own, at a date and time of my choosing, in a location and running route of my choosing, which will not have any support or security measures in place by St. Edward School, is a potentially hazardous activity, which could cause injury or death to my child. My child will not enter and participate in this virtual event unless he/she is medically able and properly trained, and by my signature, I certify that my child is medically able to perform this event, and is in good health, and is properly trained. I am fully aware that there will not be an officially marked race course, thus vehicular traffic on my child’s route will not be stopped for him/her and it is my child’s responsibility to watch for traffic and cross intersections with care. It is my sole responsibility to supervise my child(ren) throughout our chosen route. I assume all risks associated with my child running in this event, including but not limited to: falls, contact with other participants, the effects of the weather (including high heat and/or humidity), crossing intersections, traffic and the conditions of the road, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of you accepting my entry, I, for myself, on behalf of my child and anyone entitled to act on behalf of my child, waive and release St. Edward School, St. Edward Parish, the Chicago Archdiocese, the City of Chicago, event sponsors, and each of their officers, directors, representatives and successors from all claims or liabilities of any kind arising out of my child’s participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons/entities named in this waiver. I grant permission to all of the foregoing to use my child’s image in photographs, motion pictures, recordings or any other record of this event for any legitimate purpose.
